The Top 20 Hospital and Healthcare Construction Contractors in 2026
Twenty general contractors and construction managers booked a combined $25.8 billion in healthcare facility revenue in 2024, according to the most recent published ranking from Building Design+Construction. The top five alone account for $11.3 billion of that, or about 44 percent.
If you sell into hospital work as a subcontractor, supplier or specialty trade, these are the firms whose bid lists you want to be on. Below is who they are, where they are based, what they are building right now, and what each one says publicly about how it prequalifies trade partners.
How We Ranked Them
Ranking integrity matters on a list like this, so here is exactly what it rests on.
The source is Building Design+Construction's Top 120 Healthcare Facility Construction Firms for 2025, published October 27, 2025, as part of BD+C's 2025 Giants 400 Report. The measure is 2024 healthcare facility revenue. BD+C notes that the figure "factors revenue for all healthcare buildings work, including hospitals, medical office buildings, and outpatient facilities." The numbers are reported by the firms themselves in BD+C's annual survey, not audited by BD+C or by us.
We also pulled each firm's figure from BD+C's companion table, Top 100 Hospital Facility Construction Firms for 2025, which isolates 2024 hospital building revenue. That column is shown for context only. The rank order below follows the healthcare facility table exactly, positions 1 through 20, with no firms added, dropped or reordered.
A few honest caveats:
- This is a 2024 revenue snapshot. We could not find a published 2026 Giants 400 healthcare table when this article was written. When it posts, the order may change.
- Self-reported revenue means firms define their healthcare work slightly differently. Several firms reported the identical number for both healthcare and hospital revenue, which tells you they classify nearly all of their healthcare volume as hospital work.
- Some figures include joint-venture work. A few of the projects below were built by JVs that pair two firms on this list.
- Headquarters, projects and prequalification details come from each company's own website, press releases or trade press, linked inline. Where we could not verify something, we left it out.

What the List Tells Subcontractors
The prequalification pages above point to three patterns.
First, BuildingConnected and its TradeTapp module are close to the default. Robins & Morton, JE Dunn and Clark say outright that prequal runs through TradeTapp. Whiting-Turner, Brasfield & Gorrie, Skanska, Rodgers and Batson-Cook route subs through BuildingConnected in some form. If your BuildingConnected profile is stale, fix that before chasing any of these firms.
Second, COMPASS is the other shared platform. STO Building Group and Gilbane both use it, and one COMPASS form can cover both. It is a paid subscription, so weigh the cost against how many participating GCs work in your market.
Third, the paperwork is consistent across firms. Gilbane and Robins & Morton publish their lists: recent financial statements, work in progress, banking and bonding capacity, EMR, and OSHA 300 logs. Keep one current package and most of these applications become a matter of uploading files.
Hospital work also brings its own hurdles that office or warehouse jobs do not, including infection control risk assessment (ICRA) procedures, work in occupied buildings, and in California, HCAI (formerly OSHPD) plan review and inspection. Our hospital construction cost per bed breakdown covers how those requirements show up in budgets, and the hospital construction cost guide gives per-square-foot ranges you can sanity-check bids against.
Where Healthcare Construction Is Heading
The projects on this list skew toward bed towers and campus replacements: BJC's Plaza West Tower, Atrium's CMC tower, Intermountain's Billings replacement hospital, Lee Health's Fort Myers campus. These are multi-year jobs with long MEP and finish scopes, which is good news for trades that can carry a crew on one site for two or three years.
Spending data points the same way. We track the broader market in healthcare construction spending in 2026, and outpatient work, which counts toward the healthcare totals in this ranking, is covered in our medical office building cost breakdown. For one project in detail, see the Harris Health O'Quinn Hospital build in Houston. Federal hospital work is its own channel, and the VA hospital construction backlog explains why.
If you also work mission-critical jobs, there is real crossover. Several firms here, including Turner, DPR, Mortenson and Clark, also appear in our top 20 data center construction contractors. Commissioning discipline, redundant power and tight MEP coordination carry over between the two markets.
Bottom Line
Healthcare construction is concentrated. Turner alone reported more 2024 healthcare revenue than the bottom six firms on this list combined, and the top five took 44 percent of the top 20's total. But the work is spread across a lot of regional offices, and every one of these firms needs qualified trades to staff long, complex hospital jobs.
Pick the three or four firms that are actually building in your region, get your prequal package current on the platform each one uses, and ask for a smaller first scope. Price it carefully with our bid calculator and markup and margin calculator, because a clean first hospital job is how you get the second one.
FAQ
Q: What source is this hospital contractor ranking based on? A: Building Design+Construction's Top 120 Healthcare Facility Construction Firms for 2025, part of its 2025 Giants 400 Report, published October 27, 2025. Firms are ranked by self-reported 2024 healthcare facility revenue, which includes hospitals, medical office buildings and outpatient facilities.
Q: Who is the largest hospital construction contractor in the U.S.? A: Turner Construction, with $3.51 billion in 2024 healthcare facility revenue on BD+C's list. Turner reported the same figure for hospital building revenue, and DPR Construction was second at $2.01 billion.
Q: Why do some firms show much lower hospital revenue than healthcare revenue? A: BD+C's healthcare table counts all healthcare buildings, including medical office buildings and outpatient facilities, while the hospital table isolates hospital buildings. PCL, Swinerton and Clark reported hospital revenue at roughly 45, 42 and 53 percent of their healthcare totals, which suggests large outpatient and MOB portfolios.
Q: How do subcontractors get prequalified with these hospital contractors? A: Most of the firms here use BuildingConnected, often with its TradeTapp prequalification module. STO Building Group and Gilbane use COMPASS. Expect to submit recent financial statements, work in progress, bonding and banking references, your EMR and OSHA 300 logs.
Q: Will this list change in 2026? A: Probably. It reflects 2024 revenue. BD+C publishes a new Giants 400 every year, and we will update the ranking when the 2026 healthcare table is available.
