About this form
OSHA 29 CFR 1926 Subpart AA (Confined Spaces in Construction) requires a written permit before any worker enters a permit-required confined space. The permit must identify the space, document atmospheric testing, list authorized entrants and attendants, name the entry supervisor, and lay out the rescue procedure. Post the completed permit at the entry point throughout the operation, and cancel it at the end of work.
Acceptable atmospheric conditions: O2 between 19.5% and 23.5%, LEL below 10%, CO below 35 ppm, H2S below 10 ppm. Re-test continuously when conditions can change — minimum every shift for stable spaces.
